The honest answer is yes — for most of what brings people to therapy, video sessions work about as well as sitting in a room. The research on this is now fairly settled.

But ‘as effective’ is not the same as ‘identical’. Online therapy has its own strengths, its own failure modes, and one distinctly Indian complication: finding a room where nobody can hear you.

What the research actually found

Video-delivered therapy has been studied heavily over the last decade, and the pandemic accelerated that by years. Across multiple meta-analyses, therapy delivered by video shows outcomes broadly equivalent to in-person therapy for depression, anxiety disorders, stress-related difficulties and post-traumatic symptoms.

The concern researchers expected to find — that the therapeutic alliance would be weaker through a screen — largely did not materialise. Clients rate the strength of the relationship similarly across both formats. Drop-out rates are comparable, and in some studies lower online, because the practical barriers to attending are smaller.

There is a caveat worth naming: most of this research covers structured, time-limited therapy for common conditions. There is less evidence for long-term depth work or complex trauma delivered entirely online, though clinical experience there is broadly positive too.

Online is not the right first option in a few situations, and a good therapist will tell you so.

  • Acute risk. If you are actively suicidal or in crisis, you need in-person care and a support system that can be physically present.

  • Your car. Parked, engine off, windows up. Genuinely one of the most common therapy spaces in urban India.
  • Wired earphones with a mic, so only your side of the conversation exists in the room.
  • Office hours. A booked meeting room, a session before the workday, or the parking level of your building.
  • Timing around the household. Early morning before anyone is up, or the window when the house is empty.
  • Chat-based or audio-only sessions for weeks when nothing private is possible. Less ideal, but far better than cancelling.

If you are living with the people you need to talk about, tell your therapist. They can adapt — using written work, shorter check-ins, or scheduling deeper sessions for the days you can get out of the house. This is a common situation, not an unusual one.

What to check before you book

Not all online therapy is set up equally well. Ask:

  • What platform do you use? It should be encrypted and purpose-built for healthcare or at minimum end-to-end encrypted. Sessions should never be recorded without your explicit consent.
  • How are my notes stored? Encrypted and access-controlled, not in a shared drive.
  • What is your crisis protocol? A responsible online therapist will take an emergency contact and your physical location at intake, precisely because they cannot reach you physically.
  • What happens if the connection drops? There should be an agreed fallback, usually a phone call.
  • Are you qualified to practise across states? Indian therapists can generally see clients nationally, but confirm they are actually licensed and supervised.

Making online sessions actually work

A few small things make a disproportionate difference:

Treat it like a real appointment. Log in five minutes early. Do not take the session while walking, driving or half-working. The container is what makes it therapy rather than a phone call.

Put the camera at eye level. Laptop on a stack of books. It changes how present the conversation feels for both of you.

Say the awkward thing out loud. “I cannot really talk freely today, my mother is in the next room.” Your therapist can work with that.

Leave five minutes on the other side. Going straight from a difficult session into a team standup is jarring. Sit for a minute. Drink water. Let the session land.

The format matters less than the consistency. An online hour you keep every week will do more for you than an in-person hour you attend when it is convenient.
